Licensed Practical Nurse, LPN

Full Job Description

We are seeking a Licensed Practical Nurse to join our team! You will be responsible for delivering high quality care to assigned patients in a home care setting.

Responsibilities

  • Record a patient’s medical history accurately
  • Administers prescribed medications or starts intravenous fluids, noting times and amounts on patients' charts.
  • Observes patients, charting and reporting changes in patients' conditions.
  • Measures and records patients' vital signs.
  • Provides basic patient care or treatments.
  • Works as part of a healthcare team to assess patient needs, plan and modify care, and implement interventions.
  • Supervises nurses' aides or assistants.Assembles and uses equipment, such as catheters, tracheotomy tubes, or oxygen suppliers.
  • Records food and fluid intake and output.
  • Collects samples, such as blood and urine from patients, and performs routine laboratory tests on samples.
  • Prepares patients for examinations, tests, or treatments and explain procedures.
  • Helps patients with bathing, dressing, maintaining personal hygiene, moving in bed, or standing and walking.
  • Participates with RN in the coordination of care for primary patients
  • Evaluates and revises patient care goals in conjunction with the primary RN, patient and family.
  • Provides appropriate preventative, rehabilitative, and palliative nursing procedures according to the Plan of Care
  • Follows the established plan of care and notifies primary nurse or area manager in a timely manner if revision is recommended.
  • Communicates with MD, family and other involved care providers to clarify care and make aware of significant changes in condition.
  • Counsels the patient and family in meeting nursing and related goals.
  • Maintains effective working relationships with patients/families, referral sources, physicians, community resources and insurance representatives
  • Provides adequate training of patients and families
  • Assist ailing patients in daily necessary activities such as eating, bathing etc.
  • Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
  • Provide emotional and psychological support when needed
  • Communicate with patients’ family or friends to provide advice, comfort and release instructions
  • Any duties as assigned by direct supervisor

Skills

  • Previous experience in nursing or other medical fields
  • Education: Associate's degree in nursing
  • Experience: LPN license with 1-2 years of related experience
  • Current CPR/FA certification or BLS certification
  • Valid driver’s license
  • Must pass criminal background check
  • Must have an internet capable smartphone
  • Familiarity with medical tools and equipment
  • Ability to build rapport with patients
  • Compassionate and caring demeanor
  • Excellent written and verbal communication skills

Job Type: Contract

Salary: $35.00 to $42.00 /hour

Experience:

  • Home Care: 1 year (Preferred)
  • EMR systems: 1 year (Preferred)

Education:

  • Associate (Preferred)

License:

  • Licensed Practical Nurse (Required)

Work authorization:

  • United States (Required)
