Medication Management & Administration

As our loved ones  advance in age, the risk of medication errors increase. Many factor contribute to this occurrence. Older adults take  many different kinds of medication with complex dosage and  administration instructions. Mental acuteness naturally decrease with age. When someone gets sick  or injured they are prescribed new and unfamiliar medication. Discharge instruction can often be confusing. At Care demands, managing your loved one 's medication is a priority.

If you or your loved one  need it we will provide the following:

• Assessment of medication needs and status by one of our Nurses.
• Develop a home health plan and medication list
• Collaborate with your primary physician 
• Daily or weekly visit by a nurse to prepare or administer your medication
• Care workers for medication reminders
• Pill dispensers to  alert you when you need to take  your medication
